Before You Schedule
All parents and guardians must sign this Participation Agreement & Liability Waiver before their child can participate in any program at Bright Abilities & Recreational Center. Please read the full waiver below, complete the form, and submit your electronic signature. A PDF copy will be sent to your email.

Section 1 — Nature of Programs
I understand and acknowledge that all activities at BARC are recreational and enrichment-based only. BARC is NOT a medical, therapeutic, clinical, or childcare provider. Programs include, but are not limited to: dancing, painting, reading, music, yoga, arts & crafts, cooking, sensory play, and social skills activities. My child's participation is entirely voluntary and may be withdrawn at any time.
Section 2 — Health & Disclosure
I confirm that I have disclosed, or will promptly disclose, all relevant medical conditions, allergies, sensory sensitivities, behavioral considerations, and support needs to BARC staff. I confirm my child(ren) are medically fit to participate in recreational physical activities. I authorize BARC staff to seek emergency medical treatment for my child(ren) in the event I cannot be reached in a timely manner.
Section 3 — Assumption of Risk & Liability Release
I understand that recreational activities inherently carry some risk of physical injury. In consideration of my child(ren)'s participation in BARC programs, I, on behalf of myself, my child(ren), and my heirs, voluntarily assume all risks associated with participation. I hereby release and discharge Bright Abilities & Recreational Center, its owners, directors, staff, volunteers, and agents from any and all claims, demands, damages, and causes of action arising from or related to participation in BARC programs — except in cases of gross negligence or willful misconduct.
Section 4 — Photo & Media Release
I grant Bright Abilities & Recreational Center permission to photograph or record my child(ren) during activities for use in promotional materials, social media, newsletters, and program documentation. I understand I may opt out of this release at any time by notifying BARC staff in writing.
Section 5 — Attendance & Conduct
I agree to: (a) provide at least 24 hours' advance notice for cancellations or schedule changes; (b) ensure timely arrival and pick-up of my child(ren); (c) ensure my child(ren) follow BARC's behavioral guidelines, which prohibit aggressive, unsafe, or disruptive behavior. I understand that BARC reserves the right to request a parent or guardian remain on-site, or to arrange an early pick-up, if safety or participation concerns arise.
Section 6 — Drop-Off, Supervision & Pickup
I understand and acknowledge that BARC is NOT a daycare, childcare, or babysitting service. BARC provides structured recreational and enrichment programs only during scheduled session times. By choosing to drop off my child(ren), I acknowledge and agree to the following:
(a) Supervision is limited to scheduled session times only. BARC staff supervise children only during the registered program session. BARC assumes no responsibility or liability for my child(ren) before their session begins or after their session ends.
(b) Timely pickup is my sole responsibility. I agree to pick up my child(ren) promptly at the end of each session. If I am late, BARC may contact my emergency contacts, and I understand BARC is not liable for any incident occurring during any period my child(ren) remain at the facility beyond the session end time.
(c) BARC is not responsible for children left unattended. If my child(ren) are not picked up within 15 minutes of the session ending and BARC cannot reach me or my emergency contacts, BARC reserves the right to contact local authorities. A late pickup fee may apply.
(d) I am solely responsible for transportation to and from BARC. BARC does not provide transportation and is not liable for any incidents that occur during drop-off, pickup, or transit.
(e) Authorized pickup persons. My child(ren) will only be released to me or to individuals I have authorized in advance. I will inform BARC staff in writing of any authorized pickup persons.
Section 7 — Indemnification & Hold Harmless
I agree to indemnify, defend, and hold harmless Bright Abilities & Recreational Center, its owners, officers, directors, staff, employees, volunteers, and agents from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable legal fees) arising out of or related to: (a) my child(ren)'s participation in any BARC program or activity; (b) my failure to pick up my child(ren) on time; (c) any inaccurate, incomplete, or misleading information I have provided; (d) any injury or damage caused by my child(ren) to other participants, staff, or BARC property; or (e) any third-party claim related to my child(ren)'s presence at BARC. This indemnification obligation shall survive the termination or expiration of this waiver.
Section 8 — Duration & Revocation
This waiver remains in effect for one year from the date of signing or for the duration of my family's enrollment in BARC programs, whichever is shorter. This waiver may be revoked by providing written notice to BARC. BARC may require a renewed waiver if program policies change substantially.
Section 9 — Electronic Signature
By typing my full legal name below and submitting this form, I confirm that I have carefully read, fully understand, and voluntarily agree to all terms of this Participation Agreement & Liability Waiver. I acknowledge that my typed name constitutes my legal electronic signature. I understand that a signed PDF copy will be emailed to me for my records.
